View Full Version : Help please - USB, fatal ex error in IFSMGR(04)

At end of my tether now... Tried self-help on web inc MS site, I'm still
stuck.

Installing PCI card with 4 usb2 ports: inserted card, power up - BSOD,
"fatal exception 0E has occurred at 0028: C02BB3D8 in VXD IFSMGR(04) +
00012AEC".
Switched off, took card out, reboot - fine.
Did it all again - same results.

The card has Via chipset; have downloaded & installed new uscd.sys file
from MS site (following advice there) - same results.
I was trying this card because I had been getting same error when trying to
connect Kodak camera to usb port (on my Mitsu monitor, thought problem was
probably Kodak).

Running Win98SE (fine in every other respect), other usb devices (whether
connected via pc or hub built into monitor) are working fine.

Any help would be much appreciated : (

Just to update, in case it helps diagnosis....
Booted into safe mode OK, in Control Panel, Device Manager, 5 extra usb
'unknown' devices appear, althought the little usb icon thing is crossed out
in red.
Restarted in ordinary mode, back to BSOD and IFSMGR error etc. Switched
off, removed PC card with 4 usb ports.
Restarted in safe mode, in Control Panel Device Manager again, REMOVED the
unknown usb devices.
Restarted in normal mode, BSOD again.
Restarted (hitting Control) to a C:\ prompt & used WRP restore to get system
running again - we are now back to life as it was pre-usb host controller
card. : (

Would still appreciate any help you can give......
