I've had a similar problem.
In case it may help anyone, here are the details of my problem and the
solution:

I have:
- Operating System: Windows 98 SE
- CD-RW drive: TDK 121032A
- Software: Nero 5.5.3.5 (also Nero 5.0.1.3)

Symptoms:
Any time I try to burn onto a CD-R or simply try to get "CD Info", the
system crashes with a "Blue Screen of Death" (BSOD) and the message:
"Fatal exception 0E has a occurred at 0028:C003CD6A in VXD IOS(01)+
0000146E"

Problem:
Normally, when a CD is inserted into a CD drive, Windows is
automatically notified, so it can begin accessing the CD immediately
(Autorun, Autoplay, etc.) It seems that when Windows detects a new CD
in the CD-RW, the automatic notification process exposes an
incompatibility between Windows and Nero, and produces the crash.

Solution:
The symptoms go away when I disable the automatic notification. By
the way, somewhere in a readme from TDK, they recommed disabling this,
so they must know of the problem. To disable it, go to "Control
Panel", and open "System". Click on the "Device Manager" tab. Select
"View Devices by type" (at the top). Click on the "+" next to CDROM.
Click on the CD-RW drive and then "Properties". Click on the
"Settings" tab. Now, deselect the "Auto Insert Notification" (no
check mark in that box). Click on OK, OK, etc., to close "Device
Manager", and reboot the system. Once the system comes up, I can use
the CD-RW just fine, including recording and the "CD Info" feature.
The only minor negative to this is that when you insert a regular CD,
Windows won't start running automatically (no Autoplay or Autorun).

If that doesn't correct the problem for you, try uninstalling the Nero
software and re-installing it. Make sure you have handy your
"installation serial number" or "installation key number" from when
you purchased Nero. You'll need that number to re-install.

Hope this helps someone,
